Conditions for obtaining service
In case the private sector wishes to insure its employees, then all employees, their wives and children under (18) years old will be under compulsory subscription.

Procedures
- The applicant shall obtain the approved application form from the Reception Desk and fill it out.
- The applicant shall then submit the form and the enclosed documents to the Reception Desk officer.
- The form and the enclosed documents will be verified for completeness.
- If the submitted documents are not complete, the applicant will be asked to update the required documents.
- If the submitted documents are complete, the applicant will receive a payment voucher showing the due amount.
- The auditor endorses the voucher.
- The applicant pays the prescribed fees at the cashier’s counter.
- Submit the form and the enclosed documents to the Reception Desk officer
- The Applications Intake officer enters data.
- The auditor receives the application and validates information against it.
- In case of errors, the application is sent back to the Applications Intake officer to double check the entered dat
- In case the application is complete, the Audit and Posting officer will post the application.
- The application is sent electronically to printing department.
- The new card is printed and handed over to the Reception Desk officer.
- The Reception Desk officer then gives the new health card to the applicant (The representative of the institution or company)
Required Documents
- An official letter from the private sector (institution) stating its will to insure employees, attached with a list of the names of employees to be insured.
- For Jordanians, a copy of the Family Book. For nonJordanians, a copy of passport.
- One (1) personal photograph of the subscribers and beneficiaries.
Persons benefiting from the service
Jordanian and non-Jordanian employees of private sector institutions and their families whose institutions wish to insure under the government health insurance.
